To find out if you live within our practice boundary please contact reception. We offer new patients over the age of 40 a full health check with the practice nurse. Please make an appointment through the receptionists.
As a patient you have the right to express a preference to receive services from a particular health professional, and the practice will endeavour to comply with that preference, although there may be valid grounds to refuse, in which case the reason will be explained to you. For example, the person of your choice does not routinely perform the service in question within the practice.
Patients on repeat medication will be asked to attend for review at least on an annual basis
If you would like to register with the practice please find the registration forms below or alternatively call the reception to arrange collection of a registration pack. Please note that all individuals must complete both the GMS1 form and the age-appropriate practice registration form. There is also an extra form for those who are aged 18 years and over and wish to sign-up for online access.
